Schooner Memory Care is designed for individuals living with Alzheimer’s disease, dementia, or related memory challenges. As a PNMI Level IV community, we are equipped to support residents with a wide range of medical and cognitive needs while providing compassionate, person-centered care.

How do you ensure residents’ safety while promoting independence?

Our community is thoughtfully designed with secure entry and exit points, safe indoor and outdoor spaces, and clear layouts that reduce confusion. Residents can move freely within safe boundaries, allowing them to maintain independence while staff provide oversight and support.

What does a typical day look like for residents?

Every day includes a combination of social, cognitive, sensory, physical, and creative activities. Residents typically have five or more planned opportunities for engagement each day, along with one-on-one time for those who prefer it. Meals, personal care, and enrichment activities are all thoughtfully scheduled to balance structure and choice.

How is care personalized for each resident?

Each resident receives an individualized care plan based on their medical needs, routines, preferences, and life history. Our interdisciplinary team of RNs, LPNs, CRMAs, PSSs, and Life Enrichment staff collaborate closely with families to ensure care is tailored and updated as needs change.

How do you handle dietary restrictions or special diets?

Our chef works closely with a certified dietitian to meet each resident’s nutritional needs. Special diets are prepared with care and creativity, honoring taste preferences, texture requirements, and cultural or family traditions, so every meal is both enjoyable and nourishing.

Can residents continue seeing their own doctors or healthcare providers?

Yes. We coordinate with residents’ existing physicians and specialists to ensure continuity of care. Our nursing team monitors health changes and communicates regularly with families and providers to ensure all medical needs are met seamlessly.

How do you support residents who are socially withdrawn or less engaged?

We place a strong emphasis on one-on-one engagement for residents who are more reserved. Our Life Enrichment team uses personalized activities, sensory stimulation, and familiar routines to gently encourage connection, comfort, and meaningful participation.

What types of therapies or innovative tools do you use to enhance memory care?

We incorporate a variety of therapies and engagement tools, including creative arts, music, sensory stimulation, gross-motor activities, and cognitive exercises. We also use the Tovertafel interactive system, which projects games and activities onto a table to encourage movement, engagement, and social interaction.

How are families involved in the community and care process?

Families are considered partners in care. We provide regular updates, invite participation in activities, and encourage sharing of personal histories, favorite recipes, or traditions that help personalize the resident’s experience. Your insight helps us provide truly individualized care.

What should families expect during the transition to memory care?

Transitions are handled with compassion and support. We guide families through the process step by step, addressing questions, concerns, and emotional challenges. Residents are gradually introduced to the community, activities, and routines to ensure a smooth and comfortable adjustment.
