Licensed to Support Complex Needs

As a PNMI Level IV community, Schooner Memory Care is licensed to support residents with more complex medical and cognitive needs. This higher level of licensure allows us to adapt care as conditions progress, often eliminating the need for families to seek another move when additional support becomes necessary. For families, this means peace of mind knowing their loved one can remain surrounded by familiar faces, routines, and surroundings.

Our commitment to aging in place is supported by a skilled, interdisciplinary care team that includes RNs, LPNs, CRMAs, and PSSs. Through ongoing assessment and communication, our team proactively identifies changes in health, mobility, cognition, or behavior and adjusts care plans accordingly. These changes are handled thoughtfully and respectfully, with families kept informed and involved every step of the way.

Aging in place also means adapting daily support to meet residents where they are.

Assistance with personal care, medication management, nutrition, mobility, and engagement can be increased gradually and seamlessly — without disrupting a resident’s sense of routine or dignity. This flexibility allows residents to continue participating in meaningful activities while receiving the support they need to remain safe and comfortable.

We also work closely with outside healthcare providers, hospice services, and community partners when appropriate, ensuring continuity of care within the community whenever possible. These partnerships allow residents to receive additional clinical or supportive services without leaving the environment they know and trust.

Families often share that one of their greatest concerns is whether their loved one will need to move again as dementia progresses.
