Every resident receives an individualized care plan that reflects their medical needs, personal history, daily routines, and preferences. Comprehensive Assessments
The care planning process begins prior to or at the time of move-in, with comprehensive assessments conducted by our nursing and care teams. These assessments consider physical health, cognitive abilities, mobility, communication style, dietary needs, social preferences, and emotional well-being. Just as important, we take time to learn about a resident’s life story — their background, interests, habits, and what brings them comfort or joy.
Families are active partners in this process. We work closely with loved ones to gather insight, share observations, and ensure the care plan truly reflects the resident as an individual. Families often tell us how reassuring it is to feel heard and included, especially during what can be an emotionally challenging transition.
Ongoing Care
Care plans are regularly reviewed and updated to reflect changes in health, cognition, or personal preferences.
Subtle changes are often the first indicators that additional support may be needed, and our experienced staff is trained to recognize and respond proactively. Adjustments are made thoughtfully, with ongoing communication to families so there are no surprises.



Health Care Team Collaboration
Our interdisciplinary team — including RNs, LPNs, CRMAs, PSSs, and Life Enrichment staff — collaborates to ensure care plans are consistently implemented and clearly communicated across all shifts. This team-based approach helps ensure residents receive predictable, supportive care while allowing flexibility to meet changing needs.
Daily Experiences
Beyond clinical care, our care plans guide how residents experience their day — when they prefer to wake, how they like assistance provided, what helps reduce anxiety, and how they best engage socially or one-on-one. This personalized approach helps preserve dignity, reduce distress, and create a sense of familiarity and comfort.
