Memory Care Focused
Being a PNMI Level IV community allows us to deliver a higher level of care, including complex medication management, ongoing health monitoring, and support with advanced care needs — all within a secure, memory care-focused environment. For families, this means peace of mind knowing their loved one is in a setting designed to adapt as needs change.
Together, this team works collaboratively to monitor health changes, manage medications, and respond proactively to evolving needs. Communication among staff, families, and healthcare providers is a priority, allowing us to address concerns early and adjust care as needed.

Medical management at Schooner Memory Care is not just about oversight — it is about knowing each resident well, recognizing subtle changes, and responding with skill and compassion. Our structured systems, experienced team, and PNMI Level IV designation ensure residents receive safe, attentive care in a setting that feels supportive rather than clinical.
For families, this means confidence that their loved one is cared for by professionals who understand both the medical and human aspects of memory care.
Our care team includes a multi-disciplinary group of trained professionals, each playing an essential role in resident health and safety:
- Registered Nurses (RNs) provide clinical oversight, assess changes in condition, coordinate care with physicians and outside providers, and support ongoing health monitoring. They play a key role in developing and updating individualized care plans.
- Licensed Practical Nurses (LPNs) assist with hands-on clinical care, medication administration, and daily health monitoring, ensuring residents receive timely and accurate support.
- Certified Residential Medication Aides (CRMAs) are specially trained to administer medications safely and consistently under nursing supervision. Their presence ensures medications are delivered with accuracy, compassion, and respect for each resident’s routine.
- Personal Support Specialists (PSSs) provide direct daily assistance with activities of daily living such as bathing, dressing, mobility, and personal care. They are often the team members who know residents best, building trusting relationships through consistent, compassionate support.
